Background
Regenerating islet-derived protein 4, also known as REG-like protein, REG4, GISP and RELP, is a secreted protein which contains one C-type lectin domain. REG4 is involved in inflammatory and metaplastic responses of the gastrointestinal epithelium. REG4 is highly expressed in the gastrointestinal tract including the duodenum, jejunum, ileum, ileocecum, appendix, descending colon, pancreas and small intestine. It is weakly expressed in normal colon and stomach and strongly expressed in most colorectal tumors than in normal colon. It is also preferentialy expressed in mucinous tumors and in some cases neuro-endocrine tumors. REG4 protein overexpression is associated with an unfavorable response to preoperative chemoradiotherapy and may be used as a predictive biomarker clinically. REG4 is also a promising tumor marker to screen early-stage Pancreatic ductal adenocarcinoma (PDAC), and neutralization of REG4 by the antibody may offer novel potential tools for the treatment of PDAC.
